Two Laser Wavelengths –

Nd:YAG Lasers –
For Treatment of Deep-Lying Structures

Fotona’s Nd:YAG laser light penetrates deeper into the skin to selectively target even the deepest skin structures such as deep-lying veins and hair follicles. The thermal energy of the laser heats them to a point where the desired effect is achieved. With Fotona’s Q-switched technology, instead of heat, laser light is transformed into photo-acoustic waves which mechanically break apart unwanted particles, such as pigments, so that the body’s own immune system can remove them.

The effects result in an overall improvement of tightness and elasticity in the treated tissue as well as a plumping effect to the nasolabial folds from the inside, much like a filler.

FRAC3®
A self-induced, fractional effect of the Nd:YAG laser and a specific pulse structure that generates tiny regional spikes of high temperature. This nonablative modality enables the treatment of specific, deeper imperfections to complement the effect of the intraoral treatment, as well as restore youthful texture.

PIANO®
A unique, ultra-long Nd:YAG pulse mode for the third stage of Fotona4D®. The laser causes bulk tissue heating safely and rapidly, from the outside in, by concentrating energy delivery subcutaneously. The PIANO® pulse is in the superextended seconds regime mode, with a brushing technique for completing the full-thickness bulk heating with an overall synergistic tightening effect.

SupErficial™
A light cold Er:YAG ablation that gives a pearl finish to the skin. SupErficial™ additionally improves the appearance of the skin and reduces imperfections by using propriety VSP technology, enabling the operator to easily adjust the laser to an extremely controlled light peel, without thermal effects, for a no-down time, precise treatment.

What is TwinLight Laser Acne & Acne Scars Tx?
Laser treatment of Active Acne reduces acne inflammation by photo-selective absorption and heating of overactive sebaceous glands. The power of an Nd:YAG laser is used to accelerate the healing process and to reduce the possibility of developing new acne inflammation.

Laser treatment of Acne Scars is based on the precise and gentle skin resurfacing procedure where the Er:YAG laser is absorbed by the top micro-layers of the skin to vaporise scarred skin and stimulate the production of new collagen in the dermis.

How does it work?
1. Active acne: A high power Nd:YAG laser is used to penetrate the skin deep enough to thermally and selectively destroy large sebaceous glands.
2. Acne scars: The Er:YAG laser is used to remove micron-thin layers of skin so that new skin can form in its place. Unlike chemical peels and dermabrasion, laser resurfacing allows to be easily controlled. The laser gently vaporises the acne-scarred surface of the skin so that undamaged skin below is revealed.

What is NightLase® Snoring and Apnea Treatment?
NightLase® Treatment is a patented, fast, non-invasive and friendly way of increasing the quality of a patient’s sleep. NightLase® lessens the effects of sleep apnea and decreases the amplitude of snoring through the use of gentle, superficial Er:YAG laser light. No anaesthesia is used in this treatment.

How does NightLase® Treatment work?
During NightLase® treatment laser light heats the tissue, causing tightening of the tissue which leads to an attenuation of the sound of the patients snoring and a lessening of the effects of sleep apnea.

The procedure is performed in two simple steps:

What is ClearSteps Onychomycosis Treatment?
ClearSteps is a revolutionary method for treating onychomycosis in a patient friendly, quick and effective way with Nd:YAG laser light. Laser light heats evenly throughout the depth of effected nail and skin tissue, effectively weakening and killing parasitic fungi which have infected the patient’s nail.

How does ClearSteps work?
Deep, pulse-profile heating of the nail bed stimulates the killing of parasitic fungus. The natural growth and immune processes of the body are then able to restore the nail to its pristine state. Compared to traditional methods the effects of laser light spread evenly throughout the tissue and are not subject to either the limits of chemical diffusion, as are topicals, or the induction of hepatoxic side effects, as is the case with oral medications.

The procedure can be performed in three simple steps:
Step 1: File down the Nail. During onychomycosis the nail will become deformed and overgrown, it may also thicken. Filing down the nail allows laser light to fully penetrate into the treatment site.
Step 2: Administer Nd:YAG laser pulses in a circular pattern covering the entire nail. Slightly overlap the beam so that the nail is fully covered. The patient should feel a slight heating sensation throughout the nail.
Step 3: Target Nd:YAG laser pulses around the perimeter of the nail bed and around the finger or toe. Often parasitic fungus has worked its way beyond the nail bed and needs to be eliminated in all the tissue surrounding the nail to prevent recurrence.

The treatment is usually performed four times at one week intervals. All of the nails should be treated, during every treatment, just in case. Laser light can also effectively treat fungus which has infected skin tissue.

What is FRAC3® hair removal?
The 1064 nm Nd:YAG FRAC3® method is optimised to deliver high intensity laser pulses in extremely short times of less than 2 ms, sufficiently short to destroy most hair types while avoiding unnecessary damage to the epidermis. Due to the relatively low absorption in melanin, Nd:YAG laser treatments can be used safely and effectively even with darker skin types.

How does FRAC3® hair removal work?
Fotona’s Nd:YAG laser energy is absorbed selectively by the hair bulb and hair matrix, thus destroying the hair follicle. The FRAC3® pulse produces a three-dimensional fractional pattern within the epidermis and dermis, with energy being absorbed predominantly at the sites of small skin imperfections before heat is transferred to the surrounding non-target structures.

Laser parameters can be adjusted quickly and easily according to your patient’s skin type, hair thickness, hair colour as well as depth of hair follicles. Nd:YAG FRAC3® pulses should be administered with a slight overlap covering the entire treatment surface. Apply cooling to maximise comfort during the treatment.

Special post-treatment care is not required. Follicles treated in the anagen phase should fall out in approximately 2-3 weeks. Observe if new growth is present and repeat the treatment if needed. The treatment schedule will depend on each individual’s hair growth cycle.

Avalanche FRAC3® procedure for maximum comfort and safety
One of the challenges when performing laser hair removal is the relatively low absorption of laser light in the treated hair, especially when the hair is blond and/or the skin is dark. Using the Avalanche FRAC3® procedure, low energy FRAC3® laser pulses are delivered to the same skin area several times during the same session, causing a gradual increase in absorption of the laser energy in the hair. This leads to effective hair removal even at lower laser parameters than those required when single-pass hair removal is performed, making the treatment much more comfortable, often eliminating the need for cooling.

What is Laser Removal of Pigmented Lesions?
Lasers are a simple, fast, effective and safe tool for removing a wide variety of benign pigmented lesions, such as freckles, age spots, birthmarks, moles, certain vascular lesions, ephelides, melasma, seborrheic and actinic keratoses.

How does it work?
Laser removal of pigmented lesions works by destroying the pigment in the skin without the need for excision. This process in completed in 3 simple steps:
Step 1 – LIGHT ABSORPTION: The laser produces short nanosecond pulses of intense light that pass harmlessly through the top layers of the skin to be selectively absorbed by the melanin pigment in the lesion.
Step 2 – PIGMENT BREAK-UP: The laser light is transformed into photo-acoustic waves which mechanically break up the melanin pigment into smaller particles.
Step 3 – PIGMENT REMOVAL: These particles are then removed by the body’s own immune system, as the skin’s macro-phages clear the excess pigment from the area.

Why is wavelength important?
The laser wavelength is what determines which chromophore will be predominantly targeted by the laser to be selectively removed. Clinical studies show that superficial lesions can be treated most effectively with the KTP 532 nm wavelength, while the Nd:YAG 1064 nm wavelength can be used to target deeper-lying chromophores. Additionally, Q-switched light, available with Fotona’s QX MAX laser system, is especially effective due to its high peak powers and selective nature, enabling it to break apart pigments only and not cells. This means that pigment destruction can take place without ablating the skin.

Why is the QX MAX perfect for Laser Removal of Pigmented Lesions?
It is widely recognised in the medical community that Q-Switched lasers are the ideal tool for effectively removing benign pigmented lesions. Combining 4 laser modalities in an advanced, high-powered solution, Fotona’s QX MAX laser system effectively removes all common pigmented lesions.

Fotona’s innovative R-HX hand-piece produces a completely flat laser beam profile that is hexagonal in shape to allow for more precise and uniform coverage.  Homogeneity of the laser beam profile is important in pigment removal treatments and skin whitening. What is Laser Tattoo Removal?
Laser tattoo removal has become the standard treatment for tattoo removal because it offers an effective, safe and fast solution for removing unwanted tattoos. With the right laser, all common tattoo colours can be removed; black and dark tones, blues and sky blues, reds and tans, and greens.

How does it work?
The laser affects the tattoo with very short and strong laser pulses that penetrate deep into the skin, without injuring the top layers. These pulses are then absorbed by the pigments in the ink and as the laser light is transformed into photo-acoustic waves, it mechanically breaks up the ink into smaller particles. These are then naturally absorbed by the body’s own immune system.

The number of treatments depends on the size of the tattoo as well as its location, depth and colour. Amateur tattoos are the easiest to remove whereas professional ones may require more treatment sessions. The tattoo will fade slowly between treatments as the body removes the remainders of the tattoo pigment.

Why is wavelength important?
Fotona’s precisely controlled Nd:YAG laser 1064 nm wavelength is the ideal tool tattoos removal. What is IncontiLase®?

Assessments of efficacy were based on subjective patients’ evaluations through standardised questionnaires as well as objective Q-tip and PFM measurements. 90% of patients with mild SUI and 76% of patients with moderate SUI needed just a single IncontiLase® treatment. In total, 95% of all patients were incontinence free 12 months after the treatment, while 5% still had mild stress incontinence. Q-tip measurements during the Valsalva manoeuvre resulted in an average 20° decrease in Q-tip angle, objectively showing improvements in the urethra and bladder neck status.

Pelvic organ prolapse (POP) is a common problem, affecting almost 50% of parous women to some degree. How does ProlapLase® work?
The ProlapLase® treatment is based on precisely controlled laser-induced photothermal effects of Fotona’s 2940 nm Er:YAG laser in mucosa tissue, stimulating collagen remodelling and the synthesis of new collagen fibres. The result of collagen neogenesis and remodelling is shrinkage and tightening of the vaginal canal without removal of any tissue.

The ProlapLase® protocol includes three steps:
Step 1: Circular (360°) irradiation of the whole vaginal canal.

Clinical studies show that it is an efficient, easy-to-use, quick and safe procedure, eliminating the need for long-term estrogen administration.

Promising clinical Results
The latest scientific results presented by Dr. Marco Gambacciani1 and Dr. Adrian Gaspar2-4 clearly show a noticeable improvement in the symptoms of vaginal atrophy.

Results show favourable changes in the tropism of the vaginal mucosa, with increases in collagen and vascularisation as well as in the glycogen level and the epithelial thickness.

There was no ablation of the tissue or thermal damage and no adverse events.

The positive effects were long lasting as results were sustained at follow-up 6 months after the treatment. With no need for long-term estrogen treatment, RenovaLase® is also appropriate for patients with estrogen-dependent cancer (such as endometrial cancer, breast cancer, etc.) or with a family background of such cancers, as shown in clinical studies by Dr. Bojanini5 and Dr. Gambacciani2 .

How does RenovaLase® work?
The RenovaLase® treatment is based on laser-induced controlled mild heating of the vaginal tissue, which stimulates angiogenesis, fibroblast activity and new collagen formation, without thermal or ablative damage.

The final result is increased epithelial thickness as well as vascularisation of the lamina propria, along with a reduction of symptoms such as dryness, itching, irritation and dyspareunia (see figures above).
